• Dialysis device (CVVH) Description: A detailed description of the subject of the contract is given in attachment no. 4 SP Description of the subject of the order. The offered goods must meet all the items of attachment no. 4 - Description of the subject of the order, i.e. for all items in the table must be from the page the applicant's answer is "yes", or the offered value of the subject of the contract must be in accordance with the requested value of the subject of the contract by the public procurer. For the avoidance of doubt - in the case of performance parameters (where required concrete value) the applicant shall state the specific values ​​of the object of performance offered by him, from which it must be clearly evident how they meet all the requirements for the detailed required specification of the object of performance listed in column "D" of the specification table (Required value). In the case of functional parameters, the applicant selects one of the options - "yes" if the object of performance offered by him meets the given functional requirement or "no" - if the object of performance offered does not meet the functional requirement. Offer meets all the requirements for the subject of the order if the specified value of the performance parameter is within the required height/range and "yes" is specified in the case of the functional parameter. Conditions of delivery of goods and provision of services: a) The goods must be new, unused, completely refurbished and fully functional; b) handing over the object of the order without defects, including accessories; c) performing the installation of the subject of the contract, putting it into trial operation (functional test) and then into permanent operation; d) disposal packaging material and other waste related to the delivery of the subject of the contract; e) professional training of the operator on the operation and maintenance of the subject of the contract.
